Activity 1: Fill out the introductory survey that can be found in Assignments: (5 pts)
The survey asks questions such as:

Your mailing address and phone number during the semester so that I can mail exams back to you and call you if needed.

Location for you to take exams.

For help getting a proctor go to the Assessment Center's web page.

Type of calculator that you will be using. (You should have some kind of graphing programable calculator.)

Times that you will most likely be online for this class. You will need to study at least 12 hours per week and probably more like 16 hours per week to be successful in this course (even more if you are studying with distractions around you). I would like to know what days and times you most likely will be online so that I can plan my office hours accordingly.
